Neodymium:YAG laser (Nd:YAG, i.e. neodymium-doped yttrium aluminum garnet laser) is a medical device emitting laser light with a wavelength most commonly 1064 nm, less commonly 532 nm (after frequency conversion). It belongs to the group of lasers used in dermatology, vascular surgery and aesthetic medicine. Thanks to its high tissue penetration depth and high selectivity for certain skin chromophores – primarily hemoglobin and melanin – the Nd:YAG laser is widely used in the treatment of vascular lesions, pigmentation, hair removal and in procedures that improve skin texture. This technology is characterized by a relatively high safety profile and can be used in people with various skin phototypes, including patients with darker complexions.

Neodymium YAG laser – what is it used for?

The Nd:YAG laser is one of the most versatile technologies used in dermatology and cosmetology, because the wavelength 1064 nm allows deep penetration of the dermis with relatively low absorption by melanin in the epidermis. As a result, the risk of damage to the superficial layers of the skin is lower than with many other lasers, and treatments can also be performed in people with higher Fitzpatrick skin phototypes.

The most common indications for the use of the Nd:YAG laser include:

1. Treatment of vascular lesions

The Nd:YAG laser is particularly effective in the therapy of vascular lesions located deeper in the skin. The procedure is used, among others, in the treatment of:

  • telangiectasia (so-called spider veins),
  • dilated blood vessels on the face and lower extremities,
  • small reticular varicose veins,
  • hemangiomas,
  • chronic skin erythema.

The mechanism of action is based on the phenomenon of selective photothermolysis. Laser light energy is absorbed by hemoglobin present in the blood, which leads to heating of the vessel wall and its controlled closure. In subsequent weeks the body gradually absorbs the damaged vessel.

2. Removal of excessive hair

The Nd:YAG laser is used in laser hair removal. Laser energy is absorbed by the melanin present in the hair shaft and hair follicle, which leads to damage to the structures responsible for hair growth. This method enables long-term hair reduction, especially after a series of treatments.

3. Treatment of pigmentary changes

The laser can be used in the therapy of selected pigmentary lesions, such as:

  • solar hyperpigmentation,
  • lentigines,
  • post-inflammatory hyperpigmentation.

In the case of a wavelength of 532 nm, the laser energy is more strongly absorbed by melanin, which enables fragmentation of the accumulated pigment and its gradual removal by the body.

4. Acne therapy

In some protocols the Nd:YAG laser is used as part of the treatment of inflammatory acne. Thermal energy can reduce the activity of the sebaceous glands and limit the proliferation of Cutibacterium acnes, which play an important role in the pathogenesis of acne.

Neodymium-YAG laser – wide applications in cosmetology

The Nd:YAG laser has wide applications in modern cosmetology and aesthetic medicine due to the possibility of performing many therapeutic procedures with a single laser system. Different operating modes, variable pulse durations, and adjustable energy settings allow treatment parameters to be tailored to specific clinical indications.

Vascular procedures

One of the most important uses of this technology is the closure of dilated blood vessels. Thanks to deep light penetration, it is possible to effectively target vessels located in the dermis.

The Nd:YAG laser is used in the treatment of:

  • facial telangiectasias,
  • spider veins on the legs,
  • vascular rosacea,
  • small vascular lesions in the skin.

Skin photorejuvenation

In long-pulse mode, the Nd:YAG laser can be used for non-ablative skin rejuvenation procedures. Thermal energy penetrates the dermis and causes controlled heating of the tissues, which stimulates fibroblasts to produce new collagen and elastin fibers.

This process leads to a gradual improvement in skin quality, including:

  • increased firmness,
  • smoothing of fine wrinkles,
  • improved elasticity,
  • evening of skin tone.

Scar reduction

The Nd:YAG laser can be part of therapy for acne scars and post-traumatic scars. Thermal energy stimulates remodeling of the skin’s extracellular matrix, leading to reorganization of collagen fibers and improvement of scar tissue structure.

Treatment of discoloration and pigmentation

With appropriately selected parameters, the Nd:YAG laser can be used to treat certain pigmentary disorders. The mechanism of action involves the fragmentation of melanin particles, which are then removed by cells of the immune system.

Use in aesthetic medicine

In clinical practice, the Nd:YAG laser is used in many procedures performed in aesthetic dermatology and cosmetology clinics, such as:

  • laser vessel closure,
  • treatment of vascular rosacea,
  • reduction of pigmentation,
  • laser skin rejuvenation,
  • laser hair removal.

Due to its high versatility, this laser is one of the most commonly used devices in modern laser skin therapy.

Neodymium-YAG laser – effects

The effects of procedures using the Nd:YAG laser depend on the therapeutic indication, treatment parameters, and the individual characteristics of the patient's skin. In many cases, procedures require performing a series of treatments, because the biological processes occurring in the skin – such as collagen remodeling or the resorption of closed vessels – take time.

The most commonly observed effects include:

In the treatment of vascular lesions:

  • closure of dilated blood vessels,
  • gradual fading of spider veins,
  • reduction of skin redness,
  • improvement in the uniformity of skin tone.

In laser hair removal:

  • permanent damage to parts of the hair follicles,
  • reduction in the number of hairs,
  • slowing of hair regrowth,
  • thinning and lightening of remaining hairs.

In rejuvenation procedures:

  • stimulation of collagen and elastin synthesis,
  • improvement of skin firmness,
  • smoothing of fine wrinkles,
  • increased skin density.

In pigmentation therapy:

  • lightening of pigmented lesions,
  • gradual evening out of skin tone,
  • reduction in the visibility of sun spots.

Compared with some other laser technologies, the Nd:YAG laser is characterized by a relatively short recovery period. Treatments most often do not cause damage to the epidermis, so the patient can relatively quickly return to daily activities. However, appropriate selection of treatment parameters and the operator's experience are crucial for the safety and effectiveness of the therapy.
