Jawline sagging means a loss of tension and tissue support in the lower face, leading to a change in the facial oval and a blurring of the boundary between the cheek and the jawline. This process does not only affect the skin, but involves all anatomical layers of the face.
In a young face, the jawline remains clear, firm, and well-defined thanks to:
- adequate volume of fat pads,
- high collagen and elastin content,
- stable ligaments supporting the tissues,
- proper myofascial tension,
- preserved bone structure of the mandible.
With age, there is:
- degradation of collagen fibers,
- reduction of skin density,
- loss of fat tissue in the midface,
- downward displacement of tissues under the influence of gravity,
- bone resorption of the mandible and chin.
As a result, soft tissues lose support and begin to sag, especially in the area of the mandibular angle and marionette lines. This process can occur in both women and men, although in women it often develops earlier due to hormonal changes associated with a decrease in estrogen levels.

A characteristic symptom of a sagging jawline is the loss of a defined facial oval. The jawline becomes less firm, uneven, and blurred, and the lower part of the face appears heavier.
The most commonly observed features include:
- the appearance of so-called “jowls”,
- downward sagging of the cheeks,
- deepening of marionette lines,
- skin laxity under the jawline,
- disrupted proportions between the neck and the face,
- loss of definition of the jaw angle,
- excess skin in the chin area.
In some people, there is also a visual shortening of the lower part of the face and a change in the profile. The face may appear sad, tired, or heavier despite a normal body weight.
Advanced jawline sagging often co-occurs with:
- neck laxity,
- loss of cheek volume,
- loss of chin projection,
- increased gravitational aging of the face.
The changes can be particularly visible in harsh light or when tilting the head. In many cases, the first symptom is not the skin laxity itself, but the loss of support for the deep facial structures.

The process of jawline sagging is multi-level and develops gradually over many years. Facial aging does not solely consist of “skin stretching”, but involves the remodeling of the entire tissue architecture.

With age, fibroblasts produce less type I and III collagen and elastin. The skin becomes thinner, less elastic, and more susceptible to the forces of gravity.
Weakening of the retaining ligaments
The ligaments supporting facial tissues lose tension and elasticity. This causes the downward displacement of the fat pads.
Changes in adipose tissue

Years of mimic muscle activity and the effects of gravitational forces lead to the consolidation of changes in the lower face.
Sagging jawline - what accelerates it
The rate of jawline sagging depends on genetic, hormonal, environmental, and lifestyle factors. In some people, a noticeable loss of the facial contour appears as early as after the age of 35.
The most important accelerating factors include:
- intense exposure to UV radiation,
- tobacco smoking,
- rapid weight loss,
- chronic oxidative stress,
- a diet poor in protein and antioxidants,
- estrogen deficiencies,
- loss of muscle mass,
- sleep disorders,
- chronic inflammation of the body.
Also of significance are:
- thin skin structure,
- low level of facial fat,
- genetic predisposition to rapid collagen loss.
Nowadays, accelerated aging of the lower face is increasingly observed in people using drugs from the GLP-1 agonist group used in the treatment of obesity. This is because rapid weight loss leads to a sudden reduction in the volume of facial fat.

Sagging jawline and fat loss - the connection
Loss of fat tissue is one of the most important factors contributing to the deepening of jawline sagging. Facial fat tissue plays a supporting role and is responsible for maintaining youthful proportions.
In a young face, fat pads:
- cushion muscle movements,
- support the skin,
- give the face appropriate volume,
- stabilize the facial oval.
Rapid reduction of fat tissue leads to:

- hollowing of the cheeks,
- increased downward displacement of tissues,
